Terry A. Duncombe
President & Chief Executive Officer
Terry is a respected leader with more than 30 years of executive leadership and operational experience promoting quality health care in the community based care industry.
Since 2002, Terry has served as the President and CEO of Community Health Accreditation Program, Inc. (CHAP), the oldest community-based health care accrediting organization, with headquarters in Washington, DC. In her tenure at CHAP, she has successfully grown the organization into the nation’s leading community-based care accrediting body, with accreditation standards for home health, hospice, home medical equipment, pharmacy, private duty, home infusion nursing, public health, community nursing centers, and supplemental staffing.
Her past experience includes leadership roles as the Executive Director of Virtua Health, Vice President of the New Jersey Visiting Nurses Agency, whose services included home health, hospice, DME, and private duty nursing, and Administrator with Columbia HCA. In addition, she serves on numerous private and public workgroups, has authored articles on home health care issues, and regularly presents nationally on the CHAP Standards of Excellence and accreditation process.
Ms. Duncombe is licensed as a registered nurse, and holds a Bachelor’s degree in Nursing from the University of Detroit and a Masters of Healthcare Administration from Central Michigan University.

Linda Scott, RN, MSHA
Chief Operating Officer
Linda has three decades of proven leadership and experience in health care delivery and strategy, and a strong track record of achieving results, working both the private and public sectors and for the last 25 years has worked exclusively in home and community based services. She holds a Masters in Health Services Administration from The Medical College of Virginia/Virginia Commonwealth University in Richmond, Virginia and a Bachelor of Science in Nursing from Georgia State University.
Most recently, Linda served as Vice President and General Manager for Professional Healthcare Resources, Inc. (PHR) of Annandale, Virginia, a regional home health, hospice, and private duty services organization operating with eight locations in three states. Other employment includes Vice President at Johns Hopkins Home Care Group, Regional Director of Operations at Columbia/HCA Home Care Group, Administrator at Inova Home Health/Inova Health System, and Home Care Director at Nursefinders.
Linda’s programmatic experience includes performance improvement and clinical program and business development, in settings including hospital and health system entities as well as private free standing providers of home health, hospice, home medical equipment and clinical respiratory services, home infusion and ambulatory pharmacy, and supplemental staffing, personal care and private duty nursing. Linda has provided volunteer services at the Arlington Free Clinic, and earned certification in Community Health Nursing from the American Nurses Credentialing Center.
