Please note: The new CHAP CARES Online Application for Accreditation is not a registration system for the CARES Online Self Study. Agencies not currently contracted with CHAP must first complete the CARES Application for Accreditation and complete the contract process prior to accessing a self study. Agencies currently contracted with CHAP should contact CHAP Information Technology to access the online self study system.
