NAHC Highlights need for Increased Communication between Doctors and Home Health Agencies

The National Association for Home Care and Hospice (NAHC) recently highlighted the findings of a report published in the Journal of General Internal Medicine which showed the need for increased information sharing between doctors and home health providers.

Researchers used focus groups comprised of home health nurses and staff to identify problems with the coordination of care for recently discharged patients. Focus group members stated they often receive incomplete or inaccurate patient information due to the number of doctors involved in care of patients.  Specifically, the group stated the difficulty obtaining complete information from primary care physicians.

The report also highlights possible solutions including legislation to increase the responsibilities of physician’s assistants, and to increase access to electronic records.
