Pharmacy and Community-Based Health Care

Every day, patients and health care providers rely on pharmacies to dispense reliable and effective prescriptions, as well as other services and treatments. Pharmacy professionals do not always work in a traditional setting. These individuals serve their communities through independent, community-based pharmacies, mail order, online or within hospitals and long-term care facilities.   

The role of these providers involves much more than filling prescriptions, they oversee medication management, patient responses for prescribed medications and are an expert source of advice.

At CHAP, we view these providers as a critical piece of the community-based health care puzzle. Through the accreditation of pharmacies, as well as other community-based health providers we are able to bridge the gap, promoting community-wide success and delivery of the best patient care.
