2018 CHAP Standards of Excellence v.1 for Home Health Providers

The CHAP Standards of Excellence framework has been redesigned to support a customized approach to accreditation that focuses on the specific types of care and services delivered by each provider organization.

First and foremost, CHAP’s updated 2018 Standards of Excellence are designed to provide the flexibility needed by providers to deliver high quality care/services. The revisions incorporate established standards of practice and regulations, but also support a provider’s ability to demonstrate how they uniquely meet the requirements.   

CHAP also focused on current regulatory and industry changes when revising the standards.  Health care is a business that changes rapidly – from treatments to technologies to care models and everything in between. Revising both the content and the structure of our standards was necessary to support current practices as well as evolving models of care/service delivery.  CMS also issued new home health conditions of participation (CoPs) that are currently scheduled to become effective on January 13, 2018.  The 2018 CHAP Standards of Excellence v.1 incorporates the new CoPs. 

The content.

We removed outdated information, eliminated redundancies and focused on what matters most in home and community-based care.  We’ve added additional content in the areas of care planning and coordination, infection prevention and control, and quality improvement, all designed to help our partners provide the highest quality care/services to patients and their families.  We also enhanced several areas that align with CMS’ new home health CoPs, such as emergency preparedness.    

The format.

CHAP has developed a new framework to support the revised standards.  Standards are now organized by key performance area (KPAs), or topic, such as Care Planning and Coordination.  Within the framework, there are two types of requirements—standards that are more general and apply to multiple service lines, and modifiers, which incorporate service-specific and state law/regulation.  The new framework also delineates between types of standards (e.g., design and implementation). Ultimately, this will aid an organization’s ability to identify the underlying cause of problems, should they arise. Finally, we’ve taken the guesswork out of the accreditation process.  The new framework includes detailed evidence guidelines that provide information on how compliance with a standard will be determined.  



Yes. This initial release of the standards, 2018 CHAP Standards of Excellence, v.1, applies specifically to home health providers and focuses on the revised Medicare CoPs, currently proposed to become effective on January 13, 2018.  In December 2017, we will release the full set of 2018 CHAP Standards of Excellence.  By September 2018, providers in all service areas will be required to meet the revised standards to achieve accreditation by CHAP.

Over the past several years, CHAP and a team of industry experts from around the country dedicated themselves to the most significant revision to date of the CHAP Standards of Excellence.  Rather than simply updating the current service-specific standards, we developed a set of revised CHAP Standards of Excellence that are relevant across all home and community-based services. 

After a rigorous assessment of CHAP’s current Standards of Excellence, we identified areas that were missing or needed improvement when compared to national quality improvement initiatives. In doing so, we identified opportunities to streamline content, eliminate redundancy and enhance clarity.

The CHAP Standards of Excellence revision process was authorized by the CHAP Board of Directors in 2013 and was overseen and guided by the Standards Quality Board (SQB), a group of nationally recognized industry experts representing key stakeholders.  The SQB and a CHAP Board of Directors Standards task force were responsible for reviewing and approving all standards.

The 2018 CHAP Standards of Excellence v.1 for home health providers went through several periods of review by industry stakeholders and were released for public comment in January 2016.  Feedback on the content, structure, and level of burden was incorporated into the final set of standards.
